Adult Social Care Officer-Waiting List
We are seeking a dedicated and conscientious worker with prior experience in Adult Social Care, specifically with older adults, individuals with physical disabilities, learning disabilities, sensory impairments, and/or mental health issues.
Key Responsibilities:
Manage an active caseload with prior duty work/triage experience.
Engage with vulnerable adults and their carers using a strengths-based approach to help them achieve personal outcomes and access community services.
Undertake strengths-based assessments and develop support plans that meet the needs of individuals and their carers.
Work collaboratively with a diverse group of individuals and partner agencies to deliver the best outcomes.
Skills & Experience Required:
Proven experience in Adult Social Care, working with vulnerable adults.
Sound knowledge and practical experience of current legislation including the Care Act 2014, Safeguarding, Continuing Health Care, Mental Health Act, and Mental Capacity Act 2005.
A motivated, flexible approach with high professional standards.
Confidence in conducting assessments and creating support plans.
Ability to work effectively with individuals from diverse backgrounds.
